Anatomorphological Study of Normal and Infected Ethmoid Sinus by CT and MRI


Abstract
The aim of this retrospective study was to measure the ncrmal ethmoid sinus by high resolution CT and MRI, and to compare with CT of ethmoditis in Korean.
The reasons why ethmoidal investigation is important are that ethmoiditis is always associated with paranasal sinusitis, most complications of sinusitis, particularly in chidren are related to ethmoiditis, and surrounding organs including optic
nerve
may be injured during operation.
Ethmoidal measurement has been studied anatomically, but CT evaluation has not been reported. High resolution CT and MRI allowed the accurate finding of fine thin wall of ethmoidal cells than other method.

cm(SD=¡¾0.69) 1.47cm(SD=¡¾0.26), 0.44§²(SD=¡¾0.24), 1.24(SD=¡¾0.95), and 7.30(SD=¡¾1.77) in ethmoiditis on CT image.
Significant difference was noted in only longitudinal length between noramal and diseased group (P=0.0018).
2. Total area was smaller(P=0.0084) and longitudinal length was shorter9P=0.0025) in short term ethmoiditis than in long term ethmoiditis.
Wall thickening of cells were noted in 58%(35/60) of short ethmoiditis and 56%(19/34) of long term ethmoiditis, and fuil filling of fluid were in 32%(19/60) of short term and 47%(16/34) of long term ethmoiditis.
3. The correlation of mean area of each cell with total area was highest in all group(coefficiency of correlation : 0.74-0.91).
4. The lagest cells was mostly located in the posterior part of the ethmoid sinus(87.2% in normal group and 58.9% in ethmoiditis group).
5. On sagittal MRI, single layered type of cells was seen in 42%, double layered type was in 42% and multiple layered type(honeycomb shape) was in 16% of normal person. Honeycomb arrangementwas ontirely located in anterior part of the ethmoid.
Mean
height of anterior cell group was higher than that of posterior cell group(85%).
